don't worry, you can send out your tax return first and put a note about it. It takes six weeks to precess a return so you can keep looking for it or get another one  from the Quebec ministry of education. I suggest leave it blank and apply for a adjustment after you get the form. That is what I heard from the tax office. Good luck.
