You can buy them in Sears, They Bay, Wal-mart, ToysRUs, and many other baby specialty stores.
The one I use is Krushies Cloth Diaper (the one with build in cover, flap). They are sold in pack of 5 and about $50 per pack. I like them a lot. In fact, before I bought them, I did lots of research, this is the one winning the award. You can read this more in amazon.com. There are hundreds of reviews about all the brands.
They only have two size in the store, 10lb to 22lb or >22lb. If you order online from Krushies you can order premature, new born size as well.
How often to change depends on your baby. When my baby was under 3 month, I change every 2 hours. Now she's older, I change it every 4 hours max. Remember, you don't want your baby to have diaper rash because of your laziness. I still keep some regular disposable diaper for overnight and travel. Since I don't want to wake up my BB for diaper change.
If you do decided to go for cloth diaper, make sure you also buy Krushies disposable lining. Its' $6 in Wal-mart or $7 in Sears. It look like regular toilette paper, but made with 100% cotton, and specifically designed with Crushies cloth diaper. When your BB do his big job, you can just take the lining and through in the toilette and flush. Simple and no mess. It also save a lot of cleaning time.
The only down side of this cloth diaper is that you must prepare your baby bigger size cloth. The diaper is a bit bulky, so even if your baby is newborn size, the cloth you buy for him should be 3 month old cloth. Otherwise it won't fit outside of the diaper. Also, you need to wash them every 3 days. I hope you'll wash them by wash machine. It's impossible to do by hand. They are quite heavy after soak in water.
