需要填写declaration of authorization of sponsor,谁有英文版本？

谢谢。不是联邦的，是魁北克发出的，签undertaking时候附带的一个表格，在魁北克网站上，只有法语的，英文的怎么没有？原文是 Guide du Garant Demande d'engagement 和 declaration d'autorisation du garant ou du garant cosignataire

谁有上面表格的英文版？谢谢
