Medical Abortion
Medical abortion is a safe and effective alternative to surgical abortion．  It is done through the use of medication to end the pregnancy and to cause the uterus to expel the pregnancy．

The Method

There are currently two methods:

1.RU486 or mifepristone combined with misoprostol has been widely used in Europe and Asia since its development in the 1980． It is available in Canada only under the auspices of research；

2.Methotrexate combined with misoprostol米索前列醇has been used in North America since  1993 for pregnancy termination． Methotrexate氨甲喋呤has been used since 1949 and is approved for use primarily in cases of rheumatoid arthritis, psoriasis and some cancers. It has been used successfully to treat ectopic pregnancies since 1982． Misoprostol is most commonly used in the prevention of stomach ulcers．

Drug action

Methotrexate stops the process of rapid division of cells in the body：this has the effect of termination on a new pregnancy．The drug is unable to be detected after 3 days． Folates interfere with binding of the drug at receptor sites at the cell 1eve1．Making it important to avoid folate supplements and foods containing folates. Misoprosto1 (or Cytotec) causes the uterus to contract and the pregnancy to be expelled. The combined effect of these drugs is similar to a miscarriage． The effectiveness rate is 93—97％ ．



Parameters

This method is available to women who meet our criteria．Women must be in good health，eighteen years or older．Clear about their decision to terminate the pregnancy，no more than 49days pregnant by menstrual date (confirmed by ultrasound)．And able to give informed consent． They must be willing to adhere to our protocol which includes at 1east one return visit．

Comparison of the two abortion methods

Contraindications

The method is not offered to women who are：less than eighteen years old，ambivalent．Regular drug or alcohol users、not able to give informed consent，have an allergy or intolerance to methotrexate or misoprostol．Have persistent anemia， needing to leave the region of Montreal in the near future and are unable to return  for follow—up visits．Or living far from a hospita1．It will not be available to women who have no one around them to provide assistance or transportation in case of an emergency，who have a low tolerance to pain．Who  cannot abstain from sexual intercourse for the duration of the treatment(2 to 5 weeks)．Or who refuse a curettage in the event of a complication (3 to 10％ of cases) Complications are in the order of：heavy bleeding，a pregnancy which has stopped growing but is not expulsed．A pregnancy which persists．



NB: methotrexate is extremely toxic and causes severe fetal malformations in a persisting pregnancy. If a pregnancy persists，the client must agree to have a curettage

What are the side effects of the medication used?

INSTRUCT10NS

The medical abortion procedure begins with the injection of methotrexate．This is day”1”.It is important to remember that this medication is highly toxic to a pregnancy. It would cause severe malformations to the fetus if the pregnancy were to continue about 2 to 3％ of medical abortions are unsuccessfu1．If the pregnancy continues after a medical abortion，you would then have to have a surgical procedure involving dilation，aspiration and curettage



Day3，4，or5：Pick a day on which you can rest at home and when you can count on the presence of a friend or relative who can be with you

l         wash your hands wel1．With soap and water．

l         with your fingers． Insert four tablets of misoprostol deep into the vagina

l         Lie down for about half an hour. The pills may fall out after half an hour This is not a problem as all the medication will have been absorbed

l         Cramping and bleeding should start with in four to six hours．This can be heavy for the first 24 hours

l         Drink lots of liquids. Eat lightly to avoid vomiting

l         Take Triatec 30.One to two tablets every four hours for pain if you are not allergic to codeine． If this does not work，take Advil or Motrin，400 to 600 mg every four to six hours．

l         To avoid diarrhea et to reduce the abdomina1 cramps that may occur with misoprostol，we prescribe Immodium；take as needed

l         Bleeding is heavier than with a menstrual period and you may have heavy clotting. You may notice that you are passing pinkish tissue as the pregnancy is expulsed. This is normal．After several days，the bleeding lightens，and spotting may continue for up to four weeks some women have a second episode of heavy bleeding．

l         If there is little or no bleeding，repeat the dosage with four more tablets of misoprostol in the vagina 24 hours after the insertion of the first dose．

l         You should avoid sexual intercourse until the doctor has confirmed that the pregnancy has been completely terminated



Day 8 to 14: Come to your appointment whether there has been bleeding or not

药物流产麻烦，决定做人工流产，约montreal general hospital作了,这是他们给的注意事项，希望需要的人也能用上
一。事前准备：
     整个手续约3小时完成，（但夫人用了5小时，主要是在等，朋友、亲人不得进入）
    办妥手续后，护士作验血和辅导，主要咨询过敏史、避孕措施、有无生育史，生产情况（顺产还是剖腹产），询问流产原因，告知手术过程和可能出现的情况，如大出血、感染、流产不彻底等，开抗生素（去药店买，只一次，27元左右），送一种避孕药（有两种，自己选一种）。前各种同意书。
    验血是“RH”阴性，需留院观察，直到你接受特别注射才能出院（不知道为什么？）
    更衣，至少需准备一块卫生巾
二。手术过程
    上手术床，吸一种麻醉剂（口上罩上一种罩子），几分钟后进入迷糊状态。
    内部检查
    医生在病人子宫颈处注射局部麻醉剂，将特指吸管插入子宫，利用抽吸器析出胚胎。历时5-10分钟。或许会略感腹痛，是子宫抽筋的缘故
    下手术床，人迷糊，半睡眠状态，头痛、头晕、腹痛、腹胀，护士搀扶下，进休息室躺下，护士给一种东西（热的）放在腹部，约15分钟渐渐清醒，头依旧有些晕

三。手术后

   人工流产后两周，严禁有性行为
   不可灌洗阴道
   不可用止血带，只能用卫生巾
   饮食无限制，但似乎中国人讲补（象做小月子）
四。复查：
   堕胎第三周，自买试纸检测，如属阴性，因及时联系你的医生
  第四周去医生处复查
  每月定期检查，如出现下列情况，请务必联系医生：
