You have to tell the visa officer the trueth, just like what you said they have record of you in their computer. But as my experience, you will most likely be refused.
I think so. But if you won't admit you are the same person, you will have 50% chance to get the visa, if you admit it, you will be likely refused. Since they couldn't make sure you are the same person in their record.
You'd better tell the truth. The officer will check your date of birth, etc if there is a same name in there record. You will have to pay off if they know you are lying. Show some strong proof that you will be back after holiday. They may give you a one entry visa instead of 10 year visa. Good luck.
