• A duly completed Declaration of Change – Student form. This form is available at our Web site under Print Forms.
  
  
• A duly completed Change of Address card. This card is available at the financial assistance office of your educational institution.
  
  
• A letter detailing your new contact information and address.
  
  

Please send the form or information in question to the following address:
